<br />STATE OF MINNESOTA <br />GOVERNOR'S COMMISSION ON CRIME <br />PREVENTION AND CONTROL <br /> <br />APPLICATION FOR ACTION GRANT <br /> <br />PAGE 1 INSTRUCTIONS <br /> <br />1. Project Title. Enter a descriptive title, not to exceed 50 spaces. Continuation <br />projects must retain the same title used in previous years. <br /> <br />2. Program Under Which Application is Made. Specify the number and title of the program <br />under which you are seeking support. Consult the current Grant Program Information <br />booklet for program descriptions and eligibility criteria. <br /> <br />3. Beginning Date. Enter the approximate project beginning date for the grant period cov- <br />ered by THIS application. Allow for an approximately 60-day processing period from the <br />date the GCCP&C meets to determine grant awards until the project beginning date. <br /> <br />4. Duration. Specify the anticipated project duration in months for the grant period cov- <br />ered by THIS application. This application may cover up to a total of 12 months. <br /> <br />5. Sponsoring Unit. Enter the name of the state agency or local governmental unit that <br />will administer the project as direct subgrantee of the GCCP&C. <br /> <br />6. Implementing Agency. Enter the name of the agency, unit, or organization that will be <br />responsible for the day-to-day operation of this project. <br /> <br />7. County of Sponsoring Unit. Enter the name of the county in v;'hich the Sponsoring Unit <br />is located. State agencies and Indian Reservations need not ccmvlete this item. <br /> <br />8. Contact Person/Project Director. This should be someone who is knowledgeable about <br />the project and may be contacted for further information. It may be the Project Direc- <br />tor or acting Project Director if one has been selected. <br /> <br />9. Authorized Official. This is the person authorized by the sponsoring unit of govern- <br />ment to enter into binding commitments on behalf of the sponsoring <br />unit of government. For units of local government, this person must be designated in <br />the Resolution of Sponsorship, page 14. <br /> <br />10. Financial Officer. This is the person designated by the sponsoring unit of government <br />to be responsible for fiscal matters relating to the project, including accounting, <br />fund management, verification of expenditures, and financial reports. <br /> <br />11. EEO Coordinator. Each sponsoring unit of government must designate a person to <br />coordinate its affirmative action efforts and monitor its compliance with federal, <br />state, and local affirmative action requirements. <br /> <br />12. Grant numbers are located in the Grant Agreement for previous grants. <br /> <br />13. All recipients of LEAA assistance must comply with Federal guidelines and Commission <br />policy concerning Affirmative Action and Equal Employment Opportunity. These require- <br />ments are printed in the Grant Program Information booklet, and all applicants should <br />become familiar with them. In the space provided, please indicate whether the spon- <br />soring unit of government (Item 5) has a written EEO program. Also indicate whether <br />the sponsoring unit's EEO program has been certified by the state Department of Human <br />Rights, and, if certified, the date certification was granted. <br /> <br />14. Applicants must notify all appropriate A-95 Clearinghouses of intent to apply for <br />federal aid. Clearinghouses are listed in the Graut Program Information booklet. <br /> <br />15.
