ATTACHMENTA <br />Form <br />8.MissedCollections <br />TheContractorshallhaveadutytopickupmissedrecyclingcollections.TheContractoragreestopickup <br />allmissedcollectionsonthesamedaytheContractorreceivesnoticeofamissedcollection,provided <br />noticeisreceivedbytheContractorbefore11:00a.m.onbusinessday.Withrespecttoallnoticesofa <br />missedcollectionreceivedafter11:00a.m.abusinessday,theContractoragreestopickupthat <br />missedcollectionbefore6:00p.m.thefollowingbusiness <br />9. <br />TheContractormaypostponerecyclingcollectionsduetosevereweatheratthediscretionofthe <br />Contractor."SevereWeather"shallinclude,butshallnotbelimitedtocasesinwhichthe <br />temperatureat6:00a.m.isminustwenty(20)degreesFahrenheitcolder.Ifcollectionsareso <br />postponed,theContractorshallnotifytheCity.postponement,collectionwillbemadeonaday <br />agreeduponbetweentheContractorand <br />10.Hoursand <br />TheCityrequiresallsuchcollectionstobeginnosoonerthan7a.m.andshallbecomplete6p.m. <br />CollectionofrecyclablematerialsfromhouseholdsnorthofwilltakeplaceWednesdayand <br />householdssouthofwilltakeplaceeveryTuesday.Duringholidayweeks,collection <br />dayswilltakeplaceonedaylater.TheContractorrequestonetimeCityauthorizationofexceptionsto <br />thesetimeanddayrestrictionspursuanttothe"SevereWeather"section9above).TheContractor <br />mustrequestexceptionpriortotherequestedcollectioneventandspecifythedate,timeandreason <br />for <br />11. <br />TheContractorshallestablishandmaintain,inalocationapprovedbytheCity,anofficecontinuous <br />supervisionforacceptingcomplaintsandcustomercalls.Theofficeshallbeservicewithcontinuous <br />supervisionduringthehoursof8a.m.until5:00p.m.onalldayscollectionasspecifiedinthis <br />Agreement.Theaddressandtelephonenumbersofandten(10)dayspriornoticeofchanges <br />therein,shallbegiventotheCityinTheaddressofthisofficeasoftheexecutionofthecontractis <br />2828KennedyStreetMinneapolis,Minnesota <br />ComplaintsonservicewillbetakenandcollectedbytheCityandtheContractor.Thewillnotifythe <br />Contractorofallcomplaintsitreceives.TheContractorisresponsiblecorrectiveactions.The <br />Contractorshallanswerallcomplaintscourteouslyandpromptly. <br />EachmonththeContractorshallprovidetheCitywiththefollowing <br />Alistofallcustomercomplaints,includingadescriptionofhoweachcomplaint <br />Alistofalladdresseswhereeducationtagswereleftforresidentsandwhythetags <br />Alistofallmissedpickupsreportedtothe <br />Form <br />Themonthlyreportsshallbesentbymail,faxoremailtotheCity'sRecycling <br />12.CitytoSpecifyResident <br />
