City of Arden Hills Personnel Policy Effective August 26, 2003 <br /> <br />REPORTING ACCIDENTS <br /> <br />Employees are expected to adhere to all safety requirements. Whe <br />st be promptly filed and sent to the City Administrator giving f <br />the incident. <br /> <br />WORKERS COMPENSATION REPORTING <br /> <br />Workers Compensation benefits and OSHA (job safety laws) require <br />accidents, sickness, or loss of consciousness be reported as soon as possible by the employee, <br />or on behalf of a sick or injured employee, to his or her Supervisor. <br /> <br />EMPLOYEES MUST REPORT TO THEIR SUPERVISOR ALL JOB ACCIDENTS, <br />SICKNESS, OR LOSS OF CONSCIOUSNESS AS SOON AS POSSIBLE. <br /> <br />Where items of safety equipment are required by federal, state, or local rules an <br />it shall be a condition of employment that the employee wears su <br /> <br />If you are involved in an accident, remember: <br /> <br />1. All accidents involving city vehicles should be reported to the City <br />Administrat <br /> <br />2. Insist that all parties and property concerned remain at the sce <br />accident. <br /> <br />3. Obtain identifying data from the driver of the other vehicle (na <br />insurance company). <br /> <br />4. Secure names and addresses of injured persons and any witnesses <br />accident. <br /> <br />5.
