City of Arden Hills Personnel Policy Effective IM)ruary 29.20156 <br />PERSONNEL RECORDS AND FORMS <br />In order that proper reports may be made and records maintained concerning the various <br />personnel activities,the City Adffiiiistrato Director of Administrative Services shall develop, <br />or cause to have developed and installed,appropriate forms and records for this purpose.The <br />Deputy r-ere 114u..an n e^,.0 fees P4eet^,-City Clerk or the Director of Administrative <br />Services shall have responsibility for maintaining and coordinating all necessary personnel <br />records. Annually employees will be required to review the Personnel Police and sign an <br />acknowledgement form and update the Employee Information/Emergency Information <br />form. The Director of administrative Services or his/her designee shall <br />advise all employees on all personnel transactions,records systems and procedures. <br />Any employee or the employee's designated representative,when authorized in writing by the CommeOted[$15]:Devise a form for this and put in they need to <br />schedule a time. <br />employee, may review such employee's Official Personnel File maintained in the_ <br />clerk iuu ffi n Reseitif-ees T,ireeto Director of Administrative Services'office upon request to <br />the City_'dmil st± toiDirector of Administrative Services.Such review may be made during <br />regular office hours consistent with the conditions established by the City <br />Director of Administrative Services. <br />BACKGROUND CHECKS <br />All finalists for employment with the Cite will be subject to a background check to confirm <br />information submitted as part of application materials and to assist in determining the <br />candidate's suitability for the position Except where already defined by state law, the <br />Director of Administrative Services and/or City administrator will determine the level of <br />background check to be conducted based on the position being filled. <br />A final applicant for a City position will be subject to a criminal history investigation.Before <br />the investigation is undertaken,the finalist must authorize,in writing,the City to undertake <br />such an investigation on a form approved by the City attorney,and provided by the Deptt <br />Glee_iu..._..n n,.....,._ees Dir-eeto.-.Citi'Clerk. <br />8
