Has violated written rules prohibiting the use,possession,sale or transfer <br />of drugs or alcohol while working, while on City premises, or while <br />operating City vehicles,machinery or equipment;or, <br />C. Has sustained a personal injury requiring medical care, or has caused <br />another employee to sustain an injury requiring medical care;or, <br />d. Has caused a work-related accident or was operating or helping to operate <br />equipment,machinery or a vehicle involved in a work-related accident. <br />Employees will be driven to the clinic by their supervisor or the City <br />Administrator.A urine or blood sample will be taken and the sample will <br />be forwarded to a certified laboratory for testing. <br />3. An employee must notify his or her supervisor within five(5)days of any arrest <br />or conviction under a criminal drug statute.If an employee has been convicted <br />under any drug statute,he or she will be requested to pursue the requirements <br />in number 4 below. <br />4. Any employee may be required to undergo drug or alcohol testing if that <br />employee has been referred by the City for chemical dependency treatment or <br />evaluation or is participating in a chemical dependency treatment program <br />under an employee benefit plan and has been found to be chemically <br />dependent.The employee may be required to undergo testing,without prior <br />notice,during the period of evaluation or treatment and for up to two years <br />following completion of any chemical dependency treatment program. <br />5. Applicants and employees may refuse to submit to drug or alcohol testing; <br />however,a refusal to submit to required testing will be grounds for immediate <br />suspension without pay with intent to terminate.Offers of employment will be <br />withdrawn from applicants and employees who refuse testing. <br />6. Before testing is conducted,employees or applicants must state on a written <br />form if they have seen the City policy and note any over-the-counter and <br />prescription medications they are taking or have recently taken plus any other <br />relevant information. This form will be given to the hospital at the time of <br />testing. <br />7. The 4efDirector of Administrative Services or his/her designee <br />will receive the results of the drug and alcohol tests.The City will report results <br />of testing in writing to the employee or applicant within three(3)working days <br />of receipt. If the test result is positive,the City will inform the employee or <br />19
