6 <br />SECTION III: SUBMITTALS & REQUIREMENTS <br />The following structure is to be followed for the proposal submitted to the City: <br />A. COVER PAGE <br /> <br />1. Identify the name of the project <br />2. Company name, address, and main telephone number <br />3. Name and title of lead contact person with their direct telephone number and <br />email address <br />B. BUSINESS ORGANIZATION & HISTORY <br /> <br />1. Brief history of the firm, including list of principal officers, corporate structure <br />and ownership type. Identify the number of years the firm has provided similar <br />architectural and construction management services. <br />2. Identify other services the firm presently provides besides architectural services. <br />C. TEAM IDENTIFICATION <br /> <br />1. Identify key staff, consultants and positions for each of the phases proposed for <br />this work. Include brief résumé information focusing on project experience that <br />enhances qualifications for this work. <br />2. Identify project availability during the work and any project conflicts based <br />upon other work or project commitments. <br /> <br />D. APPROACH & WORK PLAN <br /> <br />1. Written narrative based on the understanding of the project scope, goals <br />and objectives. <br />2. Detailed work plan identifying major project tasks, scope of work, <br />City responsibilities and deliverables for each task. <br />3. Draft project schedule. <br /> <br />E. FEE QUOTATION <br /> <br />1. Please provide a fee proposal based on the project phases outlined in Section <br />II, Subsection B as follows: <br />a. Phase 1 – Lifecycle and Maintenance Assessment of Existing City Hall and <br />Public Works facilities: <br /> <br />i. Provide a lump sum fee proposal <br /> <br />b. Phase 2 – Conceptual Design, Site Assessment, Cost Estimation and <br />Community Engagement Facilitation for the TCAAP Civic Site: <br /> <br />i. Provide a lump sum fee proposal <br /> <br />c. Alternate 1 – Assessment of Space Needs of the Public Works Facility:
