Page 1 of 2 <br /> <br />CONSENT ITEM – 7E <br /> <br />MEMORANDUM <br /> <br />DATE: August 9, 2021 <br /> <br />TO: Honorable Mayor and City Councilmembers <br /> Dave Perrault, City Administrator <br /> <br />FROM: David Swearingen, Interim Public Works Director <br /> <br />SUBJECT: 2018 PMP Street and Utility Improvements Project – Change Order No. 8 and <br /> Final Payment <br /> <br /> Budgeted Amount: Actual Amount: Funding Source: <br /> $3,300,000 $3,137,577.20 PIR, Special Assessments, <br /> Utility Funds <br /> <br />Council Should Consider <br /> <br />Motions to approve, table, or deny the following: <br /> <br />• Approve Change Order No. 8 in the amount of $8,794.05. <br />• Approve Final Payment for the 2018 PMP Street and Utility Improvements Project in <br />the amount of $169,458.28 to Northwest Asphalt. <br /> <br />All items need a simple majority for action unless otherwise noted. <br /> <br />Background/Discussion <br /> <br />On March 26, 2018, the City Council adopted Resolution 2018-034 Awarding the 2018 Street <br />and Utility Improvement Project to Northwest Asphalt, Inc. in the amount of $2,491,595.86 <br />which since has been revised to $2,623,981.66 due to contract changes. The project reached <br />substantial completion on November 12, 2019. The final punch list items have been completed <br />by the contractor, therefore, this project is ready for close out. <br /> <br /> <br /> <br />
