APPLICATION; <br /> LICENSE. -. <br /> 1& Apl?lication. Anv person desirinq a residential kennel <br /> license required under the provisions of Section 5-75 shall make <br /> written application therefor to the City Administrator U80n a form <br /> prescribed bv and containinq such information as required by the <br /> City Administrator. Amonq other thinqs. the aoplication shall <br /> contain the followinq information: <br /> l.ll A descriotion of the real prooerty uoon which it is ~ <br /> desired to keep the animal or animals. <br /> ill The soecies and number of animals to be maintained on the <br /> premises. <br /> Ql. A statement that the apolicant/oermittee will at all <br /> times keep the animals in accordance with all the <br /> conditions prescribed bv the City Health Officer and/or <br /> Animal Control Officer, or a modification thereof, and <br /> . that failure to obey such conditions will constitute a <br /> violation of the provision of this chaoter and qrounds <br /> for cancellation'of the license. <br /> l1.l Such other and further information as may be reauired bv <br /> the City Health Officer and/or Animal Control Officer. <br /> . <br /> ----------------- <br />
