(These rates are for <br /> life only; AD&D is also available for an additional $.03/$1000.) <br /> This change will be effective July 1, 1994. Additional employee <br /> and dependent term life coverage options are also available, <br /> subject to underwriting guidelines. <br /> . Short-term disability <br /> There will be no change in the rates for the short-term <br /> disability coverage provided by Fortis through LMCIT. The <br /> short-term disability coverage provides benefits from the first <br /> day of disability resulting from injury or the eighth day of <br /> disabili~y resulting from sickness for up to 26 weeks. The <br /> disability benefit equals two-thirds of the employee's wage, to <br /> a maximum of $100, $200, or $300 weekly. Monthly premiums are <br /> $4.50 per employee for the $100 maximum; $9.00 for the $200 <br /> maximum, or $13.50 for the $300 maximum. <br /> New long-term disability coverage option <br /> A long-term disability insurance option from Fortis is now <br /> available to cities through LMCIT. The LTD coverage is <br /> available to city employees working at least 25 hours per week <br /> and earning at least $12,000 annually, with no medical <br /> underwriting requirements. There is no ~inimum group size, but <br /> if the city has 10 or fewer employees, 100% must be enrolled. <br /> For larger groups, the minimum enrollment is the larger of <br /> either ten employees or 25% of the total. <br /> The individual employee may select the monthly benefit level, <br /> subject to a minimum of $500 and a maximum of $5000 or 60% of <br /> . earnings, whichever is less.
