<br /> . ~ ---- . - ---- ~ -- - --- ---- ----- --- ---- <br />I' <br />I NORTH SUBURBAN YOUTH ASSOCIATION <br />.- <br />I GRANT APPLICATION Il\'FORMATION <br /> The policy of the NSY A is to accept grant application requests, for consideralion, from organizations <br />I and/or individuals within NSY A boundary areas. Grants are approved specifically for the purpose of <br /> capitol improvemenes, hardships, and special projects for youth related activities. <br />I The goal of NSYA, when providing funding, is to increase and maximize the number of youth served, <br /> in a safe healthy environment, in our community. NSY A acceprs fund request grant applications from <br /> youth organizations and individuals that meet youth requirements. When individuals are requesting funds <br /> [0 participate in an organized program, a representative of that organization must accompany the <br />I individual to the ineerview. Preference in approving grants will be given to individuals participating in <br /> Jocal programs. <br />I NSY A suggests that, in addition to the NSY A, organizations and/or individuals malcing application, <br /> submit the request to several other service organizations in an effort to lessen the conunitment of NSY A. <br /> If you have not applied to other Sources for funding, be prepared to explain the reason at the interview. <br />I The following information is required on thc grant application: Name of Organization/Group/lndividual <br /> Making Application; Date of Application; Coneact Name, Title, Address, Zip, Phone; Description of <br /> Organization and/or Individual Background; Financial Informalion; Statement of Need; and Amount <br />.. Requestcd (including three (3) bid proposals, if applicable. <br /> The application must be submitted thirty (30) days prior to the NSY A monthly membership meeting. <br />I Membership meetings are scheduled on lhe last Tuesday of each month. All information requested on <br /> rhe grant application must be provided to NSY A before a gram application will be considered. <br /> Government, School Districts. and their affiliates may submit graor requests. If approved, a letter of <br />I acknowledgment and acceplance must be received from the governing administration before funds can <br /> be disbursed. <br />I For action to be taken on a grant application, a representative from the organization, or the individual <br /> making the request, is required to be preseor at the NSY A membership meeting for presentalion and to <br /> answer questions regarding the request. <br />I For more information regarding the NSY A gram applicalion process, contact Bob Matson (638-4600). <br /> Mail completed grant applications to: <br />I <br /> North Suburban Youth Association <br />I 1751 West County Road B, Suite 107 <br /> RoseviIle, MN 55113. <br />I <br />,. <br />I <br />----- <br />
