<br /> . (2 ) A current rabies certificate and list of dog tags <br /> for each dog in the dwelling unit and\or kennel. <br /> (3) The species and number of dogs over the age of <br /> three (3 ) months to be maintained on the premises. <br /> (4) A statement that the applicant\license will at all <br /> times keep the animals in accordance with all the <br /> conditions prescriped by the local animal control <br /> agency, or modification thereof, and that failure <br /> to maintain and\or obey such conditions will <br /> constitute a violation of the provision of this <br /> chapter and grounds for cancellation of the <br /> license. <br /> (5) Such other and further information as may be <br /> required by the local animal control agency. <br /> (b) Investigation by the local animal control officer; may <br /> grant license. The local animal control officer shall <br /> make such investigation as is necessary and may grant, <br /> . deny or refuse to renew any application for license under <br /> this chapter and may impose conditions to the granting of <br /> the license. <br /> (c) License, Conditions. If granted the license shall be <br /> issued by the City Administrator. The license shall <br /> specify the restrictions, deemed reasonably necessary to <br /> protect any person or neighboring use from unsanitary <br /> conditions, reasonable noise or odors or annoyance or to <br /> protect the public health and safety. Such license may <br /> be modified from time to time or revoked by the local <br /> animal control officer for failure to conform to such <br /> restrictions, limitations or prohibitions. Such <br /> modification or revocation shall be effective from and <br /> after ten (10) days following the mailing of written <br /> notice thereof by certified mail to the person or persons <br /> keeping or maintaining the license. <br /> . <br />
