The place or location where the goods proposed to <br /> be sold are manufactured, stored and the manner in <br /> which goods shall be delivered to the customer; <br /> <br /> G. A photograph of the applicant, taken within sixty <br /> (60) days immediately prior to the date of filing <br /> of the application, which picture shall be <br /> approximately 2" X 2" showing the head and <br /> shoulders of the applicant in a clear and <br /> distinguishing manner; <br /> <br /> H. A statement as to whether or not the applicant has <br /> been convicted of any crime, misdemeanor, or <br /> violation of any municipal ordinance, other than <br /> traffic violations, the nature of the offense and <br /> <br /> the punishment or penalty assessed therefor; <br /> <br /> I. A list including the name, address, phone number <br /> and age (if under 18 years) of al1 persons working <br /> for or assisting in the described activity. <br /> <br /> J. In cases of transient merchant sales, the applicant <br /> must also provide: <br /> <br /> i) Proof of appropriate permission to operate on <br /> proposed site. <br /> ii) A copy of the form or individuals sales tax <br /> permit. The sales tax permit must be posted at the <br /> site; and <br /> <br /> K. The last five (5) municipalit:ies wherein the <br /> applicant has worked before coming to this City. <br /> <br /> Section 18-07 Exemptions: The provisions of this Section of this <br /> Chapter shall not apply to the following: <br /> <br /> 1. Bona fide residents of the state selling products of the <br /> farm and garden occupied and cultivated by themselves <br /> provided that such sales can be conducted without <br /> creating traffic hazards; <br /> <br /> 2. Children, age eighteen or younger, soliciting for school <br /> sponsored activities; <br /> <br /> 3. "Garage" or "Estate" sales; <br />
