At least one person shall stand by outside of the space prepared to give assistance I <br /> in case of emergency. <br /> 2. Visual, voice or signal line communications shall be maintained between the I <br /> individuals in the space and the stand-by person. <br /> 3. An approved safety belt or harness with an attached line shall be used where I <br /> practical and feasible. The line shall be at least 2000 pound test and secured <br /> outside the entry opening. I <br /> 4. The stand-by person shall not enter the confined space without alerting an <br /> emergency response team or other trained rescue workers of the intent to enter the I <br /> confined space. Stand-by persons shall not enter the space unless the stand-by <br /> person is protected by wearing a self-contained breathing apparatus (SCBA). I <br /> 5. Mechanical ventilation shall be initiated prior to entry into the space and shall <br /> continue during the entire occupancy of the space. .. <br /> 6. Continuous atmospheric monitoring shall be performed prior to entry into the <br /> space and during the entire time of occupancy of the space. I <br /> 7. Hot work is prohibited if the flammability level is 10% of lower explosive level <br /> (LEL) or greater. I <br /> 8. A confined space entry permit is reQ.Uired prior to entry. <br /> C. Class III - Immlldiately danllerous to life and health I <br /> Confined spaces where an atmosphere free of dangerous air contamination, oxygen I <br /> deficiency or oxygen enrichment cannot be verified or nrevented. Entry will not be <br /> permitted without proper authorization. I <br /> 1. Approved respiratory protective equipment (SCBA) must be worn by occupants <br /> of the space. I <br /> 2. An approved safety belt or harness with an attached line must be worn by all <br /> occupants of the confined space. The line shall be secured outside of the space I <br /> and shall be at least 2000 pound test. <br /> 3 -- <br /> I <br />
