<br /> -- - . <br /> _. <br /> I <br /> .e CITY OF ARDEN IDLLS <br /> LOCKOUT I TAGOUT WRITTEN PROGRAM <br /> I The following is the written program for lockout, or tagout, of energy sources and devices for the <br /> City of Arden Hills. The purpose of this program is to insure that machines or equipment are <br /> locked-out I tagged-out from all potentially hazardous energy before any employee(s) performs any <br /> I service or maintenance work where unexpected energization, start-up or release of stored energy <br /> could cause injury. <br /> I The following is a list of some examples of where lock-out I tag-out applies. This is by no means <br /> a defInitive list of all such sources of energy. <br /> I I. Electrical <br /> 2. Mechanical <br /> 3. Thermal <br /> I 4. Explosive (Natural Gas I Propane) <br /> 5. Hydraulic <br /> 6. Pneumatic <br /> . 7. Gravity <br /> The City shall select a responsible person to locate and identify all devices for isolating, or <br /> ~ disconnecting all sources of energy necessary to carry out this program. <br /> The res.ponsibility of the City of Arden Hills shall be to make sure that the lock-out I tag-out written <br /> I program is followed by doing the following: <br /> I. Train exposed employees in lock-out I tag-out procedures <br /> 2. Provide proper lock-out I tag-out devices. <br /> I 3. Provide proper locks and tags. <br /> 4. Enforce the use oflock-out I tag-out devices where necessary to protect the <br /> . employee(s). <br /> 5. Establish and enforce disciplinary action against any employee who, in any <br /> way, attempts to circumvent the lock-out I tag-out devices. <br /> I Trainine- ReQJlirements <br /> I. All employees will be trained in the importance of the lock-out I tag-out <br /> I standard. <br /> 2. Maintenance, and employees who work on city equipment and on the city's <br /> buildings, will be trained in the proper method to be use in the locking-out <br /> . Itagging-out of any energy sources to which they could be exposed while <br /> working on equipment and/or the buildings. <br /> 3. All new employees, both part and full time, will be trained on the lock-out I <br /> I tag-out requirements. <br /> t- I <br /> . <br />
