Transfer from one job to another, particularly to maintenance. <br /> 2. Inspection shows non-compliance. <br /> 3. Accidents show non-compliance. <br /> 4. Lock-out / tag-out programs, or standards, are updated. I <br /> 5. Additions, or changes, are made to equipment or machines. <br /> 6. On an annual basis. <br /> Lock-out / Tai-out Procedure I <br /> I. Notify all affected employees that a lock-out / tag-out is going to take place <br /> on a specific piece of equipment, machine or operation. I <br /> 2. Shut down the equipment, if it is running, by the normal shutdown procedure. <br /> DO NOT USE THE ELECTRICAL DISCONNECT TO STOP <br /> EQUIPMENT. I <br /> 3. Electrical: Pull disconnect switch to off, and install proper lock-out. <br /> 4. Mechanical: (springs, elevated machine members, rotating flywheels, etc.) <br /> must have the energy spent, or relieved, prior to lock-out / tag-out. I <br /> 5. Thermal, chemical, flanunable, pneumatic or hydraulic: (where such energy <br /> is contained in lines, or pipes) valves must be closed, lines disconnected, or I <br /> isolating blanks installed. <br /> 6. Each person working on the machine/equipment/buildiIig will place his/her <br /> lock on the locking device. -- <br /> 7. Group lock-out / tag-out will be used when more that one employee is <br /> working on anyone piece of equipmentlmachinelbuilding where they are <br /> exposed to any unwanted energy source. I <br /> 8. Stored, or residual, energy must be relieved, disconnected, restrained or <br /> otherwise made safe before work is started. <br /> Restoring locked out equipment to service will only be done when all repairs or adjustments have I <br /> been completed by all persons working on the equipment (machine or building) and all tools have <br /> been removed, all guards replaced and no one is exposed to any hazard. I <br /> Emergency removal oflock-out / tag-out, by someone other than the installer, may be done only by I <br /> an authorized person after following this procedure: <br /> I. Attempt to reach the person( s) who installed the lock( s) to find out equipment <br /> status. I <br /> 2. Review work progress to determine what has been done, and what remains <br /> to be done, to complete repairs/adjustment. <br /> 3. Advise others working on the equipmentlmachine!building that he/she is I <br /> removing the lock-out / tag-out. <br /> 4. Notify the installer, before he/she returns to work, that the lock-out / tag-out <br /> has been removed. I <br /> 2 -- <br /> I <br />
