The applicant's occupation for five (5) years preceding the date of his or her <br />application; <br /> <br />h. Whether the applicant has ever been convicted of any crime and if so, the <br /> <br /> <br />nature of such crime, the time and place at which it occurred, the date of <br /> <br />conviction, the place and length of incarceration, and such other information as <br /> <br />the Administrator may request from the applicant; <br /> <br />i. Three (3) photographs of the applicant, showing the front of his or her face <br /> <br />and each side of his or her head, not less than thirty (30) days old and at least two <br /> <br />(2) inches by two (2) inches in size; <br /> <br />j. A physical description of the applicant; <br /> <br />k. If applicant is applying for a business license to operate a massage parlor, <br /> <br />whether the applicant has had any training or experience in performing massage <br /> <br />services, and if so, the nature and extent of such experience and education and <br /> <br />including any diploma or certificate of graduation which the applicant has <br />received from an educational institution; <br /> <br />1. If the applicant is applying for a business license to operate a sauna or massage <br /> <br />parlor, or both, then a certificate from a licensed Minnesota physician indicating <br /> <br />3 - 44 <br />
