The true name, place and date of birth, street address and phone number of the <br /> <br />applicant; <br /> <br />b. Whether the applicant is a citizen of the United States; <br /> <br />c. Whether the applicant has ever used or has been known by a name other than <br /> <br />its true name, and if so, what was such name(s) and information concerning dates <br /> <br />and places where used; <br /> <br />d. The name of the business if it is to be conducted under a name or style other <br /> <br />than the full individual name of the applicant in which case a certified copy of the <br /> <br />certification required by Minn.
