Brown <br />Arden Hills City Engineer <br /> <br />Date: January 7,2004 <br /> <br />Subject: Pay Estimate #4 (Final) <br />2003 Sanitary Sewer Rehabilitation Project <br />Lift Station #5 Reconstruction <br />Lift Station #2 Reconstruction <br /> <br />Background <br />The City of Arden Hills awarded Jay Bros., Inc. of Forest Lake, MN the 2003 Sanitary Sewer <br />Rehabilitation Project on May 27, 2003 for a total contract amount of $284,788.00. <br /> <br />. <br /> <br />Project Status <br />Lift Station #5 Reconstruction <br />All work for lift station #5 has been completed. The Contractor completed the punchlist <br />for this portion of the project. <br /> <br />Lift Station #2 Reconstruction <br />All work for lift station #2 has also been completed. The Contractor completed the <br />punchlist for this portion of the project as well. <br /> <br />The project has been completed. approximately $8,000 under budget. <br /> <br />Recommendations <br />The Arden Hills City Engineer recommends the Council approve Pay Estimate #4, Final <br />Payment, for Jay Bros., Inc, of Forest Lake, MN in the amount of $22,785.33. In order to ensure <br />that all Contract papers are fully executed the City should hold onto the Final Payment until Jay <br />Bros., Inc. provides all lien waivers and the Withholding Affidavit for Contractors form to the <br />City. <br /> <br />e <br />
