Each nomination must relate to either one or all of the categories, and shall <br />include a description detailing specific cost savings, innovations, examples of exemplary <br />performance, etc. which the nominated employee has accomplished. Use additional <br />sheets of paper if necessary. <br /> <br />2). <br /> <br />3). <br /> <br />1). <br /> <br />Public Relations- This category recognizes an individual who consistently <br />projects a positive image of the City and its services to his/her co-workers <br />and the general public. <br />Exceptional Performance- Consistent high-level of dependability, <br />reliability, responsibility, and integrity over a long period of employment <br />with the City are items measured in this category. Recognition is given to <br />high quality on-going consistent dedication to the City. <br />Significant Accomplishment- Often an employee is charged with the <br />responsibility or assumes the responsibility of completing a significant <br />goal, activity, or project Frequently the completion of such a project has <br />a major impact on the operations ofthe department or the city overall. <br />This category recognizes employees for such accomplishments. <br /> <br />. <br /> <br />Please Return the Nomination Form to the Assistant to the City Administrator's <br />Office by October _,20_ <br /> <br />3 <br /> <br />. <br />
