March 1, 2006 Dear Contractor: The City of Arden Hills is interested in obtaining proposals for Tree Removal, Stump Removal, and Tree Trimming on maintaining trees located on City and privately o\vned properties and right-of-ways. Proposals must be submitted on the attached form, and must be in compliance with the following conditions: 1. The bid fmm must be fully completed and signed. The bidder should be prepared to submit a financial and experience statement upon request. 2_ It is understood that the rate shall be compensation in full for the subject work, including site cleanup and disposing of the removed tree, limbs and/or stump grindings. 3. Proposals must be accompanied by a certificate of insurance evidencing that the Contractor's equipment and employees are covered by liability and propelty damage insurance and should be kept current throughout the duration of the contract. If insurance is canceled or expired and not renewed during the contract period, the contractor must notify the City of Arden Hills. 4. Contractors shall certify to the City that they, their operators and equipment are in complete compliance with all applicable state and federal D.O.T. requirements and regulations. The contractor shan also provide the City with a brief history of the company and their professional qualifications. e 5. It is understood that payment for work performed will be made during tbe month followiug the completion of work. All bills shall be submitted to the Operations and Maintenance (O&M) Department for review and approval. 6. It is understood that all prices listed on the proposal form should be fiml for the period of April 1, 2006 thru December 31, 2006. 7. The City of Arden Hills reserves the right to reject any and/or all proposals, and to enter into agreement with separate bidders as deemed appropriate and in the best interest of the City. 8. All questions or correspondence concerning this matter should be addressed to the undersigned. Proposals should be delivered to the Arden Hills Cily Hall, 1245 West Highway 96, Arden Hills, MN 55112 by 3:00 PM on March 17,2006.
