<br />Attachment to Change Order No.4 <br /> <br />Owner: <br />Contractor: <br />Project: <br /> <br />City of Arden Hills <br />Amt Construction Company, Inc. <br />2006 Pavement Management Program <br /> <br />Description of changes: <br />Severa] additional work items were identified during City and URS staff site reviews as well as from resident comments <br />that resulted in the need for additional work during the punchlist period of the project. The items are identified as either <br />Roadway or Storm Drainage work items as indicated below. The following is an overview of the additional work items: <br /> <br />Schedule A - Roadway <br />The items listed below represent lump sum roadway work items approved by the City and performed by the Contractor or <br />Subcontractors. These work items were regarded as necessary in order to complete punchlist items and close out the job. <br />The description of the lump sum items are included below: <br /> <br />1. The concrete subcontractor replaced a curb cut that needed to be enlarged due to a granted variance and a driveway <br />apron that heaved over the winter. Both of these items were included as a part of the site review punchlist work. <br />Lump Sum Cost= $ ],1 ]2.10 <br /> <br />2. ]n the interest of working with the property owners at 3465 and 3471 Siems Court, the City asked the retaining <br />wall subcontractor to perform some additional retaining wall and paver brick work in the shared front yard of the two <br />properties. <br /> <br />Lump Sum Cost= <br /> <br />$ 1.282.00 <br /> <br />Subtotal Schedule A - Roadway <br /> <br />$ 2,394.10 <br /> <br />Schedule B - Storm Drainage <br />The items listed below represent lump sum storm sewer work items approved by the City and performed by the Contractor <br />or Subcontractors. These work items were regarded as necessary in order to complete punchlist items and close out the job. <br />The description of the lump sum items are included below: <br /> <br />1. The raingardens at 3447 and 3442 Siems Court were not functioning properly. The Contractor drained the area and <br />replaced material on the side adjacent to the roadway to allow drainage into the road subbase. <br />Lump Sum Cost= $ 565.00 <br /> <br />2. A swa]e was constructed at 3505 Ridgewood Road in order to allow for proper drainage. The Contractor graded a <br />small swale and restored the area with blanket and seed. <br /> <br />Lump Sum Cost= <br /> <br />$ 262.50 <br /> <br />3. At ]546 Arden Place an old storm outlet to Lake Johanna was removed in the back yard but the area was not <br />restored because the resident indicated that they would be doing a landscaping project in the area. That project never <br />happened, therefore the contractor restored the area. <br /> <br />Lump Sum Cost= <br /> <br />$ 868.20 <br /> <br />4. An existing storm structure in the back yard of ]553 Arden Place had a pre-existing crack, which caused erosion in <br />a new Iy constructed raingarden. As a result, the structure and rain garden needed repair. <br />Lump Sum Cost= $ 562.2] <br />
