The name of the business if it is to be <br /> conducted under a name or style other than <br /> the full individual name of the applicant in <br /> which case a certified copy of the certification <br /> required by M.S.A. Chapter 333 shall be <br /> attached to the application; <br /> e. The street addresaea at which applicant has <br /> lived during the preceding five (5) years; <br /> f. The kind, name and location of every business <br /> and occupation the applicant has been en- <br /> gaged in during the preceding five (5) years; <br /> g. The applicant's occupation for five (5) years <br /> preceding the date of his or her application; <br /> h. Whether the applicant has ever been convicted <br /> of any crime and if so, the nature of such <br /> crime, the time and place at which it occurred, <br /> the date of conviction, the place and length of <br /> , incarceration, and such other information as <br /> the clerk-administrator may request from the <br /> applicant; <br /> i. Three (3) photographs of the applicant, <br /> showing the front of his or her face and each <br /> side of his or her head, not less than thirty <br /> (30) days old and at least two (2) inches by <br /> two (2) inches in size; <br /> j. A physical description of the applicant; <br /> k.
