If the applicant is applying for a business <br /> license to operate a sauna or massage parlor, <br /> or both, then a certificate from a licensed <br /> Minnesota physician indicating that the <br /> applicant has been examined less than thirty <br /> (30) days prior to the application and has been <br /> found free of communicable diseases. <br /> (3) If the applicant is a partnership: <br /> a. The names and addressea of all partners and <br /> all information concerning each partner as is <br /> required of an individual applicant in subsec- <br /> tion (2) hereinabove; <br /> b. The name and address of the managing <br /> partner(s) and the interest of each partner in <br /> the business; <br /> c. A true copy of the partnership agreement, <br /> attached to the application, as well as a <br /> certified copy of the certification as to trade <br /> name required by M.S.A. Chapter 333. <br /> (4) If the applicant is a corporation or other <br /> organization: <br /> a. The name and, if incorporated, the jurisdiction <br /> and date of incorporation; <br /> b. A true copy of the certificate or articles of <br /> incorporation, or associai,. a agreement, and <br /> the bylaws or constitution thereof attached to <br /> the application, and, if a foreign corporation, <br /> a certificate of authority as deseribed in <br /> M.S.A.
