<br /> <br /> <br /> <br /> MASSAGE, RAP AND SAUNA PAR.LOR3 § 13.5-4 <br /> <br /> (9) Names and addresaea of all creditors furnishing <br /> credit for the establishment, acquisition, mainte- <br /> nance and furnishing of the proposed business. <br /> (10) Establishment of applicant's interest in premises <br /> by furnishing true copies of lease, deed, contract <br /> for deed, mortgage deed, mortgage, credit arrange- <br /> ment, loan agreements, security agreements and <br /> any other documents. <br /> (11) Blueprints, diagrams, plans, layouts and the like <br /> showing the construction, revision, remodeling, <br /> alteration or additiona of or to the premises and <br /> specifically showing the layout, design and <br /> arrangement of all of the rooms, halls and <br /> facilities. <br /> (12) An ownership report, prepared by a licensed <br /> abstractor, indicating the ownership of all parcels <br /> within five hundred (500) feet of the premises <br /> sought to be licensed, accompanying the applica- <br /> tion. <br /> (13) Such other information as the clerk-administrator <br /> or the council may require from time to time. <br /> (b) Personal seruice licenses; masseurs and masseuses. <br /> Applications for masaeur or masseuse licenses provid- <br /> ed for in this chapter shall be made to the city <br /> clerk-administrator on such forms as he may from <br /> time to time provide. In addition to such information <br /> as the clerk may require, the application ahall inchide <br /> or be accompanied by: ` <br /> (1) All of the information required under subsection <br /> (a)(2)• <br /> (2) Whether the applicant has ever had a similar <br /> license denied, revoked or suapended, egplaining <br /> the reasona for such denial, revocation or suspen- <br /> sion. <br /> (3) What other communities the applicant has been <br /> licensed by during the preceding five (5) years. <br /> Supp. No. 2 779 <br /> <br /> ~ <br /> <br /> ~ <br />
