<br /> <br /> <br /> <br /> ; 13.54 ARDEN HILLS CODE <br /> <br /> (4) Written proof of age in eacess of eighteen (18) <br /> years. <br /> (5) A diploma or a certificate of graduation from the <br /> educational institution at which the applicant <br /> received inatruction on the theory, method, or work <br /> of massage, if applicant has received one. <br /> (6) A certificate from a licensed physician that the <br /> applicant has been esamined lesa than thirty (30) <br /> days prior to the application and has been found <br /> free of communicable diseases. <br /> (7) Name and address of the licensed massage <br /> establishment by which the applicant egpects to <br /> be employed. <br /> (c) Verifications. All applicationa for licenses, whether <br /> businesa or personal service, shall be verified as to all <br /> information contained therein being taue and conect. <br /> If the application is made by a natural person, it shall <br /> be signed and the verification sworn to by such <br /> person; if by a corporation, by an officer thereof; if by <br /> a partnership, by one of the partnera thereof; and if by <br /> an unincorporated association, by the manager or <br /> managing officer thereof. <br /> (d) Changes in application information. It ahall be the <br /> continuing duty of each licensee promptly to notify the <br /> city clerk-administrator of any change in the informa- <br /> tion or facts originally furniahed or required to be <br /> furnished in or along with the application for the li- <br /> cense. This duty ahall continue throughout the period <br /> of such license, and failure to comply with this sub- <br /> paragraph shall constitute cause for revocation, sus- <br /> pension or nonrenewal of such license.
