Applicat ons for masseuw or masseuse lieenses <br /> pr ded for s,n this Ordinance shall be Ynade to the <br /> City Clerk-Administrator on such forms as he may frQm <br /> time tv time provide e In acldition to such information <br /> as the Clerk may require, the application sha]ll <br /> include or be accompanied by: <br /> (1) All of the inforndation required unclex <br /> SuUsection (a) (2) o <br /> <br /> (2) 4hether the applicant has ever had a <br /> similar license denied, revoked or <br /> suspended, explaining the reasons-fog <br /> such denial, revocation or suspension. <br /> (3) TnThat other communities the applicant <br /> has been .licenseci by.during the pre- . <br /> . cedinq five (5) years e <br /> <br /> (4) Written proof of age in excess of 18 <br /> years. . <br /> (5) A diploma or a certificate of graduation <br /> ' from the educational institution at which <br /> the applicant received instruction on <br /> the theory, method, or work of massage-- <br /> if applicant has,received oneo <br /> <br /> (6) A certificate from a licensed physician <br /> that the applicant has been examined <br /> less than thirty (30) days prior to the <br /> application and has been found free of <br /> comanunicable diseases. ' <br /> (7) Name and address of the licensed massage <br /> establishment by which the applicant <br /> expects to be e.naployed. <br /> (c) Verific.ations. All apgalications for licenses, <br /> whether business or personal service, sha.ll be verified <br /> as to all information containecl therein being true and <br /> correct. If the application is made by a natural person, <br /> it shall be signed and the verifieation sworn to by such <br /> person; if by a corporatiorn, by an officer thereof; if <br /> by a partnership, by one of the partners thereof; ancl if <br /> by an unincorporated assocation, by the manager or <br /> managing officer thereof. <br /> (d) Changes in Application Information. It shall be <br /> the continuing duty of each licensee promptly to notify <br /> the City Clerk-Administrator of any change in the <br /> information or facts oraginally furnished or required to <br /> be furnished in or along with the application for the <br /> license, This duty shall continue throughout the period <br /> of such license, and failure to comply with this sub- <br /> paragraph shall constitute cause for revocation, suspen- <br /> sion or non-renewal of such license. <br /> Section 5.
