MEMORANDUM <br /> TO: Jeffrey Schultz and Victoria Byrne <br /> S& B Center Corp, dba Kelly's Korner <br /> FROM: City of Centerville <br /> DATE: March 9, 2011 ' <br /> RE: Conditions for 2011 Iiquor licenses issued to S& B Center Corp/ Kelly's Korner <br /> On March 9, 2011, the Centerville Gity Council approved the issuance of on-sale and off- <br /> sale liquor licenses for 2011 for S& B Center Corporation, dba Kelly's Korner at 7098 <br /> Centerville Road, Centerville, Minnesota. Issuarice of those licenses a,re subject to conclitions set <br /> I forth by the City Council. Those license conditions are stated in this memorandum and are in <br /> .. <br /> . . <br /> addition to any other condihons requ�red by law. The Licensee must acknowledge the license <br /> i conditions and accept responsibility for meeting the license conditions. If for any reason the <br /> license is not issued by March 31, 2011, then this approval expires and applicant may thereafter <br /> reapply and such sha11 be considered a new license application. License issuance is contingent <br /> upon Licensee paying outstanding taxes, utilities, and provide proof of insurance before <br /> 03/31 /2011. <br /> As a condition of holding liquor licenses (on-sale and off-sale) for 2011, the licensee and <br /> any responsible party of the licensee must meet the following conditions: <br /> a. No changes in the operation or ownership of the business. <br /> b. Licensee must immediately report (within 48-hours) any criminal conviction of <br /> licensee or its corporate officers. <br /> c. Licensee must fully and accurately disclose any information requested by the <br /> City. <br /> d. Any of the issues listed above will be grounds for a public hearing for immediate <br /> license revocation by the City. <br /> e. Licensee to pay outstanding taxes as well as city utilities by their respective due ' <br /> dates. <br /> f. Licensee shall secure insurance as required by city code and other law before , <br /> 03/31/20ll and shall thereafter pay premiums when due, such that insurance ' <br /> remains in continuous effect. Further, Licensee sha11 provide proof of such <br /> payment in the form of a Certificate of Insurance issued to the City of Centerville. <br /> g. Licensee agrees to reimburse City within 30 days of billing for costs of <br /> monitoring license conditions. <br /> h.
