12 -9 -2011 <br /> CITY OF CENTERVILLE <br /> PERSONNEL POLICIES <br /> 8. PERSONAL TIME OFF (PTO) - LEAVE WITH PAY <br /> 8.1 General <br /> Eligibility: Full-time Employees who have worked for the City for at least six <br /> months are eligible to accrue annual Personal Time Off leave (PTO) from the date <br /> the Employee is hired. PTO will be available for use as it is accrued according to the <br /> schedule below. <br /> 8.2 Accrual <br /> Rate of Accrual: Full-time Employees shall earn PTO leave at the following rates. <br /> Years of Service PTO Earned <br /> 0 through 5 years 6.77 hours per pay period <br /> 6 through 10 years 8.31 hours per pay period <br /> 11 through 15 years 9.85 hours per pay period <br /> 16 through 20 years 11.38 hours per pay period <br /> After 20 years 12.92 hours per pay period <br /> Accrual While Using: Employees using earned PTO shall be considered to be <br /> working for the purpose of accumulating additional PTO. <br /> 8.3 Requirements <br /> Usage: Personal Time Off (PTO) Leave may be used as earned, subject to approval <br /> by the Department Supervisor or City Administrator. PTO may be taken in <br /> increments of one hour or more. Up to a maximum of 208 hours of accrued PTO <br /> may be carried over to the next year. <br /> Procedure: To be eligible for PTO leave with pay, an Employee shall report as <br /> soon as possible to his or her Department Head the need to take personal leave and <br /> its estimated duration. Requests to use PTO shall be received at least twenty -four <br /> hours in advance of the requested time off. This notice may be waived at the <br /> discretion of the City Administrator. <br /> Pay -Out of PTO Leave: The rate of PTO pay will be the Employee's regular rate <br /> of pay in effect at the time of PTO request. In the event of termination in good <br /> standing, all earned PTO will be paid at the Employee's regular rate of pay. <br /> Page 1 of 35 <br /> Adopted 12 -28 -2011 <br />
