Potentially Dangerous Dog Requirements <br /> Centerville/Circle Pines/Lexington <br /> (Circle One) <br /> 1. The owner shall provide and maintain a proper enclosure. <br /> 2. If the dog is outside the proper enclosure the dog must be muzzled, restrained by <br /> a substantial chain or leash (not to exceed 6 (six) feet in length), and under the <br /> physical restraint of a person 16 years of age or older. <br /> 3. The owner of a potentially dangerous dog must annually register the dog with the <br /> City and be issued a Certificate of Registration stating that the animal is <br /> potentially dangerous. <br /> 4. The owner must provide and show proof of microchip identification as required <br /> by state law. The name of the microchip manufacturer and identification number <br /> of the microchip must also be provided to the Animal Control Officer. <br /> ***The Centennial Lakes Police Department may seize any dangerous or potentially <br /> dangerous dog if the owner does not meet each of the above conditions within 14 <br /> (fourteen) days.*** <br /> If you intend to appeal this matter to your city council, please sign and return this form <br /> to the Centennial Lakes Police Department within 14 (fourteen) days.
