INSURANCE PLANS <br /> <br />11.1 Health Insurance <br /> <br />Upon proper application and subject to payment of any required premiums, all <br />regular full time Employees, working at least 32 hours per week, will be required to <br />be covered by the City's health, dental and life insurance plans. Subject to the <br />payment of the required premiums, an Employee's dependents may also be covered <br />under such plans. <br /> <br />11.2 Cafeteria Plan <br /> <br />(A) <br /> <br /> <br />ong term care insurance <br /> <br />The Employer will contribute an amount, designated 0 <br />of the City Council, to the Employee for use . <br />Employer. Time to attend <br />required training will normally be paid time. <br /> <br />Advance payments may be authorized for conference registration fees. Once paid, <br />Employees will be held responsible for those fees if the Employee later chooses not <br />to attend, unless unforeseen circumstances appear such as an illness, death in the <br /> <br />24 <br />
