APPENDIX C CITY OF CENTERVILLE <br /> C. Qualification Statement Requirements <br /> Information regarding service delivery must be provided to demonstrate that the Contractor has the <br /> ability to meet the requirements herein. Actual information must be provided when preparing <br /> statements; any estimates that are made must be accompanied with information that serves as the <br /> estimate's basis. <br /> Please demonstrate in the qualification statement by use of reference or example, the Contractor's <br /> ability to successfully fulfill the following service requirements: <br /> 1. General Requirements. The minimum characteristics of a comprehensive solid waste <br /> collection program shall include: <br /> a. Weekly garbage, refuse collection, transport and disposal for all single family through <br /> four unit-residential dwellings in the City; <br /> b. Weekly (April through November) Yard waste and brush collection transport, and <br /> disposal/processing for all single family through four-plex residential dwellings in the <br /> City that elect to use the service; <br /> c. At least bi-weekly recyclable collection, transport and processing for all single family <br /> through four-plex residential dwellings in the City. Recyclables to be collected include: <br /> newsprint, glass, aluminum steel bi-metal and tin cans, corrugated cardboard, mixed <br /> paper, plastic containers, and household appliances and large items (per fee schedule in <br /> Appendix B). <br /> 2. Customer Service: <br /> a. The Contractor shall demonstrate their commitment to customer service and show how <br /> they plan to resolve customer service issues and/or disputes. <br /> b. The Contractor shall discuss how they intend to maintain an office equipped with <br /> telephones and staffed with sufficient personnel to handle complaints, inquiries, and/or to <br /> receive instructions. Full-time oversight of the City's collection must be provided. <br /> 3. Reporting Requirements: <br /> a. The Contractor must be capable of providing the City with certified weight receipts <br /> and/or quantities for the monthly collection of all solid waste (garbage and refuse), yard <br /> waste, appliances, and other recyclable materials. To substantiate the accuracy of the <br /> Contractor's procedure, the City may, at its discretion and at no additional cost to the <br /> City,require the Contractor to separately weigh each material. <br /> b. The Contractor must be capable of reporting the type and amount of material and where <br /> the materials will be delivered. If the Contractor delivers materials to a broker, the <br /> Contractor must report what end market the broker used. Summaries by brokers and end <br /> markets also must be available to the City. Brokers and processors receiving materials <br /> from these programs must be in compliance with the state and local government permit <br /> and licensing laws. <br /> c. The Contractor shall provide a summary of all complaints that have been received and <br /> how they have been resolved, to the City in an electronic format on a weekly basis. <br /> 4.
