Marketing material examples provided <br />Deadline: Proposals must be submitted no later than <br />Request for Proposals <br />Background <br />111zq 4,z at 4:00 PM. <br />The City of Centerville's goal is to offer a preferred vendor to our residents to provide education <br />and reduced rates for insecticide injections for treatment of those trees that will remain viable <br />to continue growth. <br />With increasing levels of infestation apparent throughout our community, it has become critical <br />that the City assist residents with this issue in fashion possible to stream -line the process for <br />residents seeking injections to protect their privately -owned ash trees and offering them <br />education regarding possible infestation and ramifications. <br />The City currently has no program in place for providing treatments for trees on private <br />property. <br />PROPOSER INFORMATION <br />Company Details <br />A proposer must be a properly insured, respected, financially viable company that has been in <br />business for at least five (5) years, with at least three (3) years of experience performing plant <br />health care or injection treatments for local units of government or other public agencies. There <br />must be at least one ISA certified arborist available to supervise the program and the application <br />crew(s). All members of the application crew must be licensed commercial pesticide applicators. <br />Each proposer must provide a list of three (3) municipal or public agency references. <br />Tree inspections <br />Tree inspections shall be performed prior to recommending treatment to residents. These <br />inspections must be performed by a person who is at minimum, a Certified MN Tree Inspector <br />or an ISA Certified Arborist. Individuals recommending treatment must be qualified to identify <br />emerald ash borer in the field and have experience evaluating tree condition, particularly as it <br />relates to determining which trees are suitable candidates for injection and which are not. <br />Treatment of trees exhibiting the following conditions must be noted and reported to the City <br />upon inspection, treatment or none treatment: <br />• More than 30% canopy decline <br />• Signs of severe decay in trunk or major branches <br />• Major cracks in trunk or major limbs <br />
