• Fungal fruiting bodies present on trunk <br />INJECTION DETAILS <br />Chemical <br />• Injections must be with a liquid formulation of 4% emamectin benzoate <br />• All label directions for chemical product used must be followed. <br />• At a minimum, trees less than 22 inches DBH shall be treated with 2.5 ml of 4% <br />emamectin benzoate solution/inch DBH, trees greater than 22 inches DBH shall be <br />treated with 5 ml of 4% emamectin benzoate solution/inch DBH. <br />Residents' trees <br />Injections performed on residents' privately owned trees will be driven by public demand, so <br />numbers of treatments for these trees will not be available at this time. Residents shall <br />undertake treatments with you if desired but will solely be for their privately owned trees. <br />Private Property <br />The homeowner will contact the Contractor directly to initiate an inspection, receive a quote, <br />and provide permission. The Contractor shall inspect the ash trees on private property and <br />determine whether they are appropriate for treatment. <br />GuarantPP <br />Two-year warranty required for all private injections. If a tree is killed by EAB during this two-year <br />period, or if canopy loss attributable to EAB becomes equal to or greater than 50% during this same <br />period with active beetles or larvae found, a 100% refund will be required. <br />Cost Proposal <br />Proposers shall fill out the included cost and rate forms for private tree treatments. Proposed fee <br />schedule shall include the included tables as well as any additional fees, costs for <br />consultation/assessment visits with homeowners, and taxes. <br />Listing <br />Contractors will provide a listing of all privately owned tree(s) treated by them to the City on a monthly <br />basis. <br />Education and Outreach Materials <br />Contractor shall provide outreach materials, such as a postcard or pamphlet to all residents to inform <br />them about the program. The outreach materials shall be sent in the mail. Contractors shall include <br />both costs for this mailing and examples of previous outreach materials used for similar programs with <br />proposal.
