<br />(2) Whether the applicant has previously been denied a tattooing, body piercing, body branding or <br />body painting license from any other governmental unit; <br /> <br />(3) The location of the business premises and the legal description thereof; <br /> <br />(4) Whether all real estate and personal property taxes that are due and payable for the premises to <br />be licensed have been paid, and if not paid, the years and amounts that are unpaid; <br /> <br />(5) If the application is for premises either planned or under construction or undergoing substantial <br />alterations, the application must be accompanied by a set of preliminary plans showing the <br />design of the proposed premises to be licensed. A copy of the plans must be submitted to the <br />Building, Fire and Health authorities for approval. If the plans of design are on file with the city, <br />additional plans need not be submitted; <br /> <br />(6) For each person or business entity named in the application, disclose whether they have been <br />convicted of, or charged with a felony, gross misdemeanor or misdemeanor, including traffic <br />violations, and if so, the date and place of conviction, the nature of the offense, and identifying <br />information such as a ticket number or case number; and <br /> <br />(7) Other information as the City Administrator may require. <br /> <br />(E) Written Authorizations. Each person or business entity named in or as part of this application shall <br />execute a written authorization allowing the city and/or the Chief of Police to verify all information <br />disclosed as part of the application. <br /> <br />(F) False statements. It is unlawful to knowingly make a false statement in the license application and is <br />punishable as a misdemeanor criminal offense. In addition to all other penalties, the license may be <br />denied or subsequently revoked by the City Council for a violation of this section. <br /> <br />Ord. #71, adopted 11/13/2002 <br /> <br />§ 114.05 APPLICATION EXECUTION. <br /> <br /> An application for a license must be signed and sworn to. If the application is that of a natural person, it <br />must be signed and sworn to by that person; if that of a corporation, by an officer thereof; if that of a <br />partnership, by one of the general partners; and if that of an unincorporated association, by the managing <br />officer.
