<br />3. Identification of Qualifications of Assigned Personnel <br /> <br />a. The name of the person who will be responsible for the <br />management and administration of the comprehensive planning <br />process with a resume describing that person's experience and <br />qualifications. <br /> <br />b. The names and resumes of the professional staff that will be <br />assigned to providing services to the City. <br /> <br />c. An organizational chart identifying team members and their areas <br />of responsibility. <br /> <br />d. A statement of committing the aforementioned staff to this <br />proposal. <br /> <br />e. Background information concerning the firm, including number of <br />years in business under this name and breakdown of personnel in <br />the proposing office. <br /> <br />f Information on similar Comprehensive Plan projects completed by <br />the firm including contacts who can be used for references. <br /> <br />4. Respondent's Detailed Approach to the Scope of Services <br /> <br />a. The proposal shall address, in detailed fashion, the approach of the <br />firm. <br /> <br />5. Basis for Compensation <br /> <br />a. The proposal shall include a detailed cost estimate for completing <br />the project. Work done under the contract for services for the <br />Comprehensive Plan shall be billed on a time and material basis <br />against a not-to-exceed budget. Work produced under the contract <br />shall be the property of the City ofCenterville. <br /> <br />6. Potential Conflicts <br /> <br />a. Potential conflicts must be disclosed. <br /> <br />7. Disclosures and Assurances <br /> <br />a. Applicant Authority - Assurance that the signatory making <br />representations in the proposal, on behalf of the Respondent, has <br />the authority to do so. <br /> <br />b.
