<br />c. Ro1es and responsibi1ities of individua1s invo1ved in <br />project imp1ementation: Arc employs 11.0 FTE professional staff <br />whose efforts are augmented by well over 200 volunteers. The <br />Anoka office houses 6.6 FTE professional staff with 1.5 <br />designated for adult services and Toy Library. <br /> <br />A summary of staff expertise is as follows: <br /> <br />Marianne Reich - Reich has over twenty years experience with <br />Arc chapters and has worked at Arc Anoka since 1993. Reich has <br />experience providing advocacy and public policy development <br />services as well as background in financial and agency management <br />and board development. She is responsible for completing the <br />CDBG application, signing the contract and submitting <br />reimbursement requests. <br /> <br />Celeste denDaas - denDaas has been the Family Services Director <br />for three years. As Director of Family Services she provides <br />support opportunities, information and referral and <br />workshops/educational programs for families who have children <br />with developmental disabilities. She also provides technical <br />assistance on the needs of families and appropriate toys and <br />adaptive equipment for the library. <br /> <br />Stacey Torborg- Torborg has been the Adult Services Coordinator <br />for four years at the Anoka office. She has a social work degree <br />and over seven years experience serving adults with significant <br />disabilities in day habilitation and residential programs. <br />Torborg provides individual advocacy assistance to adults, <br />facilitates the People First Self Advocacy Group and provides <br />information and referral services. <br /> <br />II. Project Funding: <br /> <br />A. Amount of request: Arc is requesting $1,000 from the City of <br />Centerville CDBG Funds. <br />B. CDBG funds compliment other funding received from United Way, <br />community groups and organizations, corporate and foundation <br />grants, and agency fund raising events to allow us to provide <br />services to families and adults with disabilities. <br />c. Total cost of this program to Anoka County residents is <br />D. Total CDBG funds received last year were $13,000. <br /> <br />E. Funding includes: <br />United Way of Minneapolis $219,260, confirmed for 2000 <br />Other United Ways $164,000 confirmed for 2000 <br />Special event fundraising $84,950 <br />Contributions from individuals/businesses $197,500 <br />Corporate & Foundation grants $149,736 - confirmed $75,290 <br /> <br />3 <br />
