<br />III. Project Requirements: <br />A. Detai1ed project description: <br /> <br />Goa1s and Services: <br /> <br />Goal 1: Adults with developmental disabilities and/or their <br />guardians will indicate that services received by Arc helped to <br />maintain or increase opportunities to be active participants in <br />their community. <br /> <br />Services: 5-10 adults with developmental disabilities living in <br />Centerville will receive information and/or one to one assistance <br />to help them live, work, and participate in the community. <br />Activities to include People First Self Advocacy group - monthly <br />meetings, special events, metro-wide conference, annual Holiday <br />Party. Individual advocacy to help resolve problems or increase <br />leisure opportunities. <br /> <br />Evaluation: Adults receiving services or their guardians when <br />appropriate are given an opportunity to evaluate the quality and <br />effectiveness of services received. Agency goal is 90% of <br />issues/problems/concerns identified are resolved satisfactorily. <br />Information is compiled on types of assistance requested, <br />individual outcomes, and trends and issues that develop as a <br />result. <br /> <br />Goals 2: Families report that the Toy and Adaptive Equipment <br />Lending Library was easy to use and had toys and equipment <br />available that met their individual needs and <br /> <br />3: Through the use of toys and equipment, children will learn <br />new strategies or skills to help them increase motor, <br />speech/language, and cognitive development. <br /> <br />Services: 3-5 families in Centerville will use the Toy and <br />Adaptive Equipment Library. Maintain adequate supply of toys and <br />increase supply of adaptive equipment to meet individuals needs <br />addressed by families. <br /> <br />Evaluation: All families using the library complete an evaluation <br />form yearly indicating if/how the library helped their family and <br />children. Families needing assistance completing the evaluation <br />are helped in person or over the phone. <br /> <br />B.
