Without CDBG funds the Outreach Worker is only able to enter the area for Energy Assistance applications or for crisis situations, thereby dealing with crisis intervention rather than crisis prevention. Detailed Project Description The project activity consists of a Senior Outreach Worker identifying and meeting with vulnerable and isolated seniors and disabled adults, and assisting them in obtaining financial, social, medical and nutritional services. These may include food programs, Public Health Nursing, chore services, Medical Assistance, crisis home repair, and others. The Outreach Worker also works with community groups, senior centers, city staff and other service providers to identify appropriate clients and to determine the most appropriate resources available to them.
