Why are CDBG funds necessary for this project? <br /> <br />Without CDBG funds the Outreach Worker is only able <br />to enter the area for Energy Assistance applications or <br />for crisis situations, thereby dealing with crisis <br />intervention rather than crisis prevention. With <br />funding, the Worker would be able to serve <br />significantly more clients, in addition to seeking out <br />clients, providing more extensive service. <br /> <br />C. Total estimated cost of project. <br /> <br />Total estimated cost of the project is $1,022.00 <br /> <br />D. Amount of past CDBG funds used for this project. <br /> <br />During the 1998-1999 CDBG funding cycle, this program <br />received $ 765.00 from the City of Centerville. <br />No CDBG dollars were received for our 1999-2000 program <br />year. <br /> <br />E. Other funding being used <br /> <br />1. Provide a list of all anticipated funding <br /> <br />Other funding utilized by the Senior Outreach <br />Program includes Energy Assistance Administrative <br />dollars when the outreach worker completes Energy <br />Assistance Applications for clients, and county <br />dollars when crisis situations are indicated. ACCAP <br />also continues to work with all communities to <br />solicit private sources which would supplement CDBG <br />funding. <br /> <br />2. Provide status of anticipated funding sources <br /> <br />County and Energy Assistance limited funding has <br />been secured. <br /> <br />III. PROJECT REQUIREMENTS: <br /> <br />A. Detailed Project Description <br /> <br />The project activity consists of a Senior Outreach <br />Worker identifying and meeting with vulnerable and <br />isolated seniors and disabled adults, and assisting <br />them in obtaining financial, social, medical and <br />nutritional services. These may include food <br />programs, Public Health Nursing, chore services, <br />Medical Assistance, crisis home repair, and others. <br />The Outreach Worker also works with community groups, <br />senior centers, city staff and other service providers <br />to identify appropriate clients and to determine the <br />most appropriate resources available to them. <br /> <br />ANOKA COUNTY COMMUNITY ACTION PROGRAM.
