<br />Frequently, the Outreach Worker is contacted by a <br />community group or a business interested in providing <br />assistance to those in need, and requests help from <br />the Outreach Worker in identifying such residents who <br />would benefit from such programs as Christmas baskets <br />or house painting. <br /> <br />Finally, the Outreach Worker meets every two weeks <br />with the four other Outreach Workers in Anoka County <br />and the Director of Senior Programs. This meeting <br />provides support, education, information and resource <br />development for the staff. The Outreach Worker also <br />receives daily supervision and guidance from the <br />Director of Senior Programs at ACCAP. <br /> <br />B. What is the CDBG National Objective <br /> <br />Low/Moderate Income Limited Clientele <br /> <br />C. What makes the project eligible for CDBG funds? <br /> <br />The Centerville Senior Outreach Program is eligible <br />under the Public Services category. <br /> <br />D. What is the regulation number that applies to National <br />Objective being served? <br /> <br />570.201 <br /> <br />IV. PROJECT BENEFIT: <br /> <br />A. How does the project benefit the low/moderate <br />residents of Anoka County? <br /> <br />Persons who benefit from this program are seniors and <br />disabled adults in Centerville, particularly those <br />who are housebound or low/moderate income. In <br />addition, the entire business and residential <br />community benefits when clients are able to remain in <br />their own homes, and can continue to contribute <br />economically and socially to the community. This <br />program helps prevent premature nursing home <br />admission, resulting in lower financial costs for all <br />community members. Finally, county offices and other <br />service providers who do not normally have adequate <br />access to isolated residents are able to provide <br />services by utilizing the one-on-one contact that the <br />Outreach Worker provides. <br /> <br />1.
