<br />Section 7: Section 112.03(A)(1)(h) and (A)(5) of the Centerville City Code is hereby amended to read <br />as follows: <br /> <br />(A)(1 )(h) Whether applicant has ever been convicted of, or charged with a felony, gross <br />misdemeanor or misdemeanor, including traffic violations, and if so, the date and place of conviction, <br />the nature of the offense, and identifying information such as a ticket number or case number; <br /> <br />(A) (5) Written Authorizations. Each person or business named in or as part of this application shall <br />execute a written authorization allowing the city and/or the Chief of Police to verify all information <br />disclosed as part of the application. <br /> <br />Section 8: Section 113.03 (C), (C)(1), and (C)(2) of the Centerville City Code is hereby amended to <br />read as follows: <br /> <br />(C) Application. Application for a city license to conduct business as a peddler or transient merchant <br />or to be registered for contribution solicitation shall be made at least 14 days before the applicant <br />proposes to conduct the licensed or registered activity. Application shall be made on forms supplied by <br />the city. The application shall include the names and addresses of all persons that will be conducting <br />the licensed or registered activity, prior locations where similar activities have been conducted, the <br />dates that the activities are proposed to be conducted and the other information and the city may require <br />as a condition oflicensing or registration. All applicants must disclose whether applicant has ever been <br />convicted of, or charged with a felony, gross misdemeanor or misdemeanor, including traffic <br />violations, and if so, the date and place of conviction, the nature of the offense, and identifying <br />information such as a ticket number or case number. <br /> <br />(1) Written Authorizations. Each person or business named in or as part of this application shall <br />execute a written authorization allowing the city and/or the Chief of Police to verify all <br />information disclosed as part of the application. <br /> <br />(2) False statements. It is unlawful to knowingly make a false statement in the license <br />application and is punishable as a misdemeanor criminal offense. In addition to all other <br />penalties, the license may be denied or subsequently revoked by the City Council for a violation <br />of this section. <br /> <br />Section 9: Section 114.04(D)(6), (E) and (F) of the Centerville City Code is hereby amended to read as <br />follows: <br /> <br />(D) In the case of all applicants: <br /> <br />(6) For each person or business entity named in the application, disclose whether they have been <br />convicted of, or charged with a felony, gross misdemeanor or misdemeanor, including traffic <br />violations, and if so, the date and place of conviction, the nature of the offense, and identifying <br />4 <br />
