<br />3. Identification of Qualifications of Assigned Personnel <br /> <br />a. The name of the person who will be responsible for the <br />management and administration of the company's <br />services with a resume describing that person's <br />experience and qualifications. <br /> <br />b. The names and resumes of the professional staff that <br />will be assigned to providing services to the City. <br /> <br />c. An organizational chart identifying team members and <br />their areas of responsibility. <br /> <br />d. A statement of committing the aforementioned staff to <br />this proposal. <br /> <br />e. Background information concerning the firm, <br />including number of years in business under this name <br />and breakdown of personnel in the proposing office. <br /> <br />f. Information on similar projects completed by the firm <br />in the last five years, including contact names who can <br />be used for references. <br /> <br />4. Basis for Compensation <br /> <br />a. The proposal shall include a detailed cost estimate for <br />completing the identified work, including a detailed <br />estimate of services and costs associated with each <br />task. Hourly costs for additional time should also be <br />provided for each staff member who would be assigned <br />to work. <br /> <br />5. Potential Conflicts <br /> <br />a. Potential conflicts of interest must be disclosed. <br /> <br />6. Disclosures and Assurances <br /> <br />a. Applicant Authority - Assurance that the signatory <br />making representations in the proposal on behalf of the <br />proposer has the authority to do so. <br /> <br />b. Insurance Coverage - Documentation of current <br />insurance coverage and limits, including professional <br />liability insurance shall be provided with the proposal. <br /> <br />IV. SCOPE OF SERVICES <br /> <br />The City of Centerville is requesting proposals from qualified firms with considerable <br />experience in financial planning, tax increment plan writing, redevelopment financing, <br />grant writing, grant administration and debt issuance.
