<br />IV BENEFITS. <br /> <br />A. Eligibility. All regular employees shall become <br />eligible for and may receive certain benefits, as set <br />forth below. An employee suspended without pay for <br />more than one pay period, or on a regular unpaid leave <br />of absence, shall not be eligible for any benefits and <br />shall not be allowed to use any benefits other than <br />group health insurance, life or long term disability <br />insurance as provided for in the insurance policies. <br />Part-time employees who do not regularly work 30 hours <br />or more per week and temporary employees are not <br />eligible for City benefits other than the continuing <br />education benefit. <br /> <br />B. Insurance. Long term disability insurance is provided <br />to all employees. Life insurance is provided to all <br />employees after thirty (30) days of continuous service. <br /> <br />Major medical and hospitalization insurance are <br />provided to all qualified regular employees after <br />thirty (30) days of continuous service. An employee is <br />required to carry individual coverages, however an <br />employee may select indi vidual, two-party, or family <br />coverage. <br /> <br />The City shall make a specified contribution per month <br />toward the cost of the coverage, and this contribution <br />may differ depending on the type of coverage selected. <br />The difference between the actual cost of the selected <br />coverage and the City's contribution shall be deducted <br />from the employee's paycheck. For those employees <br />selecting individual coverage, the amount of any <br />employer contribution over the cost of medical <br />insurance may be used for qualified insurance premiums, <br />eligible flex spending expenses or taken as taxable <br />compensation subj ect to a maximum set by the employer <br />annually. The City may choose to segregate retired <br />employees from other employees for pooling purposes in <br />determining the premium for insurance as allowed by <br />state law. <br /> <br />Part-time employees who regularly work 30 hours or more <br />per week shall receive pro rata insurance benefits. <br />Suspended employees and employees on a short leave of <br />absence without pay shall receive insurance benefits as <br />if they were not suspended or on leave. Employees on a <br />regular unpaid leave of absence shall receive insurance <br /> <br />IV-l <br />
