<br />(D) Any period of absence to uceive multiple treatments by health cate <br />ptoviders fot reconstructive smgety after an accident at injury, ot for a <br />condition that would likely tesult in a period of incapacity of more than three <br />(3) consecutive days if left untreated. <br /> <br />Medical Certification: The City may requite the Employee to docmnent the need <br />for the leave by providing a certification issued by a health cate provider. The <br />Employee will be allowed fifteen (15) calendar days to obtain the certification. <br /> <br />Intermittent/Reduced Schedule Leave: Employees may take leave on an <br />intermittent basis or work a reduced schedule undet certain circumstances and <br />subject to City Council teview and approval. <br /> <br />Substitution of Paid Leave: Employees may choose to use accrued paid leave to <br />cover some or all of the leave taken. <br /> <br />Job Restoration: When an Employee returns from medical leave, the City will <br />restore the Employee's original job or an equivalent job unless the EmplQyee's job <br />was eliminated during the leave. In this instance. the employee will be treated as laid <br />off putsuant to Section 5.4. <br /> <br />Health Beuefits: The City will keep the Employee on its health insurance coverage, <br />including family coverage, but will not continue to pay the City's share of the <br />coverage. The Employee must pay the entire premium. The City may cancel <br />coverage if the Employee's ptemium payment is mote than thirty (30) days late and <br />the City has given the Employee written notice at least 15 days in advance advising <br />that coverage is going to be canceled if the ptemium is not teceived. <br /> <br />Other Benefits: Other benefits, including cash payments chosen by the Employee <br />instead of gtoup health insmance coverage do not have to be maintained during <br />periods of unpaid medical leave. <br /> <br />9.3 Military Leave <br /> <br />General: Minnesota Statutes, ~ 192.26 - 192.261 ptovide that an Employee of a <br />municipality who is a member of the National Guard, the Naval Militia, the Officer's <br />Reserve Corps, the Naval Reserve, the Marine Corps, or any other reserve <br />component of the military or naval fotces of the United States, is entitled to a leave <br />of absence without loss of pay, seniority status, efficiency rating, or benefits for the <br />time such Employee is engaged in training Ot active service, not exceeding a total of <br />fifteen (15) days in any calendar yeat. <br /> <br />Conditions: The leave of absence is only in the event the Employee returns to <br />employment intmediately upon being relieved from military ot naval service, or is <br />prevented from returning by physical or mental disability or other cause not the fault <br />of the Employee, or is requited by the proper authority to continue in military or <br /> <br />20 <br />
