No more than two (2) ~assaqe therapist individual <br />licenses will be issued to any business establishment. <br /> <br />3. Licenses ~ay be denied or rescindecl if the premises of <br />the establishments do not meet the requir~ent. of the City <br />CounCil, and of the building, safety and sanitation resulations <br />of the City at Little canada, and State of Minnesota. <br /> <br />4. Licenses may be denied or rescinded if there is any <br />fraud or deception involved in the license application. <br /> <br />5. LicenseS may be denied or rescinded if the applicant, <br />licensee O~ e~plQyee of the $ame fail to comply with, or have a <br />history of violations of the laws or ordinances which ap~ly ~Q <br />health, safety, or moral turpitude. <br /> <br />6. The city Council ~ay attach suoh reasonable conditions <br />to the license as it, in its S014 disoretion, ~.ems to be <br />appropriate. <br /> <br />7. The City may run criminal history Checks on any <br />applicant for a license pursuant to this chapter. <br /> <br />2202.040. MASSAGE THERAPY ONLY BY LICENSED PF~ONS. <br /> <br />1. No person shall provide massage therapy without being <br />licensed individually as providod in this chapter. <br /> <br />2. Application for license. Any person desirinq to he <br />licensed Shall file an application on forms provided by the City <br />Administrator. The application shall contain s~ch information as <br />tn~ city Administxator may require, including: <br /> <br />a. The applioant's full namQ, address, social <br />security nu~er, and written proof ot aqQ. <br /> <br />b. The name and address of the establishment by which <br />the applicant expects to be employed. <br /> <br />c. A statement concerning whether the person has been <br />oonvioted of or entered a plQa ot ~ilty to any <br />crime or ordinance violation and, it so, <br />intor.ma~ion as to the time, place and nature of <br />such orime or offense. <br /> <br />d.
