Personal service licenses. <br /> <br />a. an the information required under section 810.020(1), subparagraphs b, e, g, h, and j; <br /> <br />b. the location or locations where the applicant will perform massage services; <br /> <br />c. photographs of the applicant taken by the city of Minnetonka' s police department <br />showing both the front and side view; and <br /> <br />d. a complete set of fingerprints taken by the Minnetonka police department. <br /> <br />3. Massage therapists. A massage therapist applying for a personal service license need not <br />provide photographs and fingerprints but must instead provide verification of meeting the <br />requirements specified in section 810.005(10). <br /> <br />4. Subsequent changes in license application information. A licensee must promptly notify <br />the director of community development of a change in the information or facts required to <br />be furnished on the application for a license, even after the license has been issued. <br /> <br />. <br /> <br />810.025. Execution of Application. An application for a license, whether business or <br />personal service, must be signed and sworn to. If the applicant is a natural person, the <br />application must be signed and sworn to by that person; if a corporation, by an officer of the <br />corporation; if a partnership, by one of the partners; and if an unincorporated association, by <br />the manager or managing officer of the association. <br /> <br />810.030. Application Fees. <br /> <br />1. An application for a license must be accompanied by the license fee specified in section <br />710. If an application is rejected, the city will refund the amount paid. No other refunds <br />will be made. <br /> <br />2. An application for a business license must be accompanied by an investigation fee in the <br />amount specified in section 710. No investigation fee will be refunded. <br /> <br />810.035. Persons Ineligible for a License.
