<br />September23,2009 <br /> <br />Hatem Qamhieh <br />7251 Clearwater Drive, <br />Centerville, MN 55038 <br /> <br />Honorable Mayor and City Council <br />City of Centerville <br />1880 Main Street <br />Centerville, MN 55038 <br /> <br />Dear Mayor and coundl: <br /> <br />I appreciate your time to let me present to you what is going on Clearwater Drive in Centerville since <br />the construction started on 2009 5treet and Utility Improvement Project #000616-07158-0. <br /> <br />On July 15, 2009, I contacted Dallas larson by phone to let him know what is going on and what <br />Forest lake Construction, Inc. is doing on Clearwater Drive (Please see the email I sent to Dallas <br />larson on Friday, July 17, 2009). <br /> <br />On Friday, July 31, 2009, Oty Engineer Mark 5tatz called me back after several attempts to connect <br />with each other. I discussed with him the situation on Clearwater Drive and informed him that Forest <br />lake Construction, Inc. using the Cul-De-Sac as full staging area for their construction resulted in <br />total damage for the road surface. Mark mentioned to me that he had visited the Clearwater Drive <br />two weeks before our conversation and he is going to send a letter to every body on the cul-de-sac <br />that next year they will do Fog and Seal for the road. I told him that the damage is ten times worse <br />since your observation two weeks ago and I recommended for him to do mill and over lay for the <br />road. Mark responded that it will be too expensive for the contractor to do that. <br /> <br />Forest lake Construction, Inc. staged the project on the cul-de-sac with poor coordination from the <br />beginning of the project. They excavated the middle of the cul-de-sag and the hydrant location eight <br />times. <br /> <br />Now we are at the end of September and the damage on Clearwater Drive is above and beyond since <br />my conversation with Mark on July 31. The Forest lake Construction, Inc. kept using the cul-de-sac <br />as staging area for their project from driving their heavy equipments and parking th~m on the road <br />such as bulldozers, scrapers, compactors, hauling trucks, water trucks which all have 'very high axel <br />load. They used the road to store their aggregates, such as sand, class 5 aggregate, top soils and <br />sand resulting in scraping the surface of the pavement when they try to use the material for the rest <br />of the project. Also they stored the stonm sewer pipes on the cul-de-sac. <br /> <br />I addressed only the basic things related to the problem.
