Rate of Accrual: Full -time Employees shall earn PTO leave at the following rates. <br /> Years of Service PTO Earned <br /> 0 through 5 years 6.77 hours per pay period <br /> 6 through 10 years 8.31 hours per pay period <br /> 11 through 15 years 9.85 hours per pay period <br /> 16 through 20 11.38 hours per pay period <br /> After 20 years 12.92 hours per pay period <br /> Accrual While Using: Employees using earned PTO shall be considered to be <br /> working for the purpose of accumulating additional PTO. <br /> 8.3 Requirements <br /> Usage: Personal Time Off (PTO) Leave may be used as earned, subject to approval <br /> by the Department Supervisor or City Administrator. PTO may be taken in <br /> increments of one hour or more. Up to a maximum of 192 hours of accrued PTO <br /> may be carried over to the next year. <br /> Procedure: To be eligible for PTO leave with pay, an Employee shall report as <br /> soon as possible to his or her Department Head the need to take personal leave and <br /> its estimated duration. Requests to use PTO shall be received at least twenty -four <br /> hours in advance of the requested time off. This notice may be waived at the <br /> discretion of the City Administrator. <br /> Pay -Out of PTO heave: The rate of PTO pay will be the Employee's regular rate <br /> of pay in effect at the time of PTO request. In the event of termination in good <br /> standing, all earned PTO will be paid at the Employee's regular rate of pay. <br /> Use of PTO Leave during a Holiday: Holidays occurring during an Employee's <br /> PTO leave will be considered a holiday and the Employee will not be charged for PTO <br /> on that day. <br /> Coordination of PTO Leave with Family and Medical Leave: In order to allow <br /> coordination of Personal Time Off leave with Family and Medical Leave, Employees <br /> seeking to take PTO may be required to provide enough information about the purpose <br /> of the leave to allow a determination of whether the leave will also qualify for Family <br /> and Medical leave. <br /> Waiver of PTO Leave Prohibited: No Employee is permitted to waive personal <br /> leave for the purpose of receiving double compensation. <br /> Workers' Compensation: Employees are covered by workers' compensation laws <br /> and regulations of the State. In the event an Employee is disabled and is entitled to <br /> workers' compensation, the Employee will keep any workers' compensation <br /> payments received and will be eligible to receive a bi- weekly pay check equal to one- <br /> third (1/3) of the Employee's regular pay through the use of personal leave benefits. <br /> Page 17 of 51 <br /> Adopted XXX, 2009 <br /> 65 <br />
