APPENDIX 13 <br /> INSURANCE <br /> Effective January 1, 1999: <br /> The EMPLOYER will continue to pay fifty percent (50 %) of the individual health <br /> insurance premium for permanent part -time employees up to a maximum of one <br /> hundred fifty dollars ($150.00) per month. <br /> The EMPLOYER will pay a flat fee of two hundred dollars ($200.00) per month <br /> for permanent full -time employees electing single health insurance coverage. <br /> Any amount over the $200.00 premium will be paid by the Employee. Any <br /> excess amount not used for health insurance under the $200.00 flat fee can be <br /> used to purchase additional life insurance or taken as deferred compensation. <br /> The EMPLOYER will pay a flat fee of three hundred fifty dollars ($350.00) per <br /> month for permanent full -time employees electing family insurance coverage. <br /> Any amount over the $350.00 premium will be paid by the Employee. Any <br /> excess amount not used for health insurance under the $350.00 flat fee can be <br /> used to purchase additional life insurance or taken as deferred compensation. <br /> The EMPLOYER shall continue to provide for each employee the same Dental <br /> Insurance coverage as current non - bargaining unit employees receive in <br /> accordance with the City's current policy. <br /> Effective January 1, 2000: <br /> In contract year 2000, Unionized employees will receive the same <br /> adjustment per category as the non - unionized employees. <br /> 18 <br />
