Infrastructure If the project application is requesting assistance for water /sewer improvements, <br /> / Yes street improvements, or other infrastructure, answer the following questions: <br /> ❑ No 1. Identify the urgency of the improvement. City of Centerville wants all residents <br /> to be connected to the public water supply by 2012. This project allows for low <br /> and moderate income residents in the City of Centerville to hook up to the City <br /> watermain in order to get onto City water in advance of the deadline. <br /> 2. Is the improvement included in the city /town improvement plan? Yes <br /> 3. What circumstances led to the need for federal assistance? Low and moderate <br /> income residents need assistance in order to comply with the 2012 deadline. <br /> Slum /Blight If this project is for slum /blight removal, verification must be made in order to <br /> Removal assure that a CDBG- funded activity meets the national objective regarding slums and <br /> blight.
