In the case of a license renewal, the clerk has deterMined that the applicant is operating in good. standing <br />with the city by: <br />1. Veri.fying.that all written complaints received from .residents or business owners of the <br />.city regarding the applicant's service under the terms and conditions of the license <br />have been resolved or are likely to be resolved in the near future.. <br />2. Verify. ing.the. applicant has performed to the. standards and conditions of the: license. <br />(7) The city clerk shall issue a new license, ❑r transfer own ers€iip. of a license, upon completion. of <br />the following: <br />a. Receipt from the applicant of all required forms and applications,. fully completed. <br />b. Receipt of the requi.red.proof of insurance,..as required under section 62-58. <br />c. Receipt from the applicant of the required license fee. <br />d. In the case of an initial application by a new licensee, said licensee shall be. required to <br />deposit a investigation fee in the amount established by the city council,. <br />e. Corn pletion of a background investigation report determining the applicant's abiIity to <br />provide adequate and reliable service under the conditions of this division. <br />f. Approval of license. issuance, or transfer of ficense,'by the city council.. <br />(Prior Code, s 105=0.90). <br />Sec, 62-58.. - Insurance requirement. <br />An applicant must obtain all. the insurance required by this. section. All costs. and expense are the <br />responsibility of the applicant. All insurance coverage issubjectto the approval by the city. For a license to <br />remain valid, insurance coverage must be maintained by the Iice. nseed.uring the term of the license. No <br />work under this license shaII be comrnenced unti€ the following. requirements have been satisfied: <br />(1) The licensee shall obtain and maintain statutory workers' compensation insurance and <br />employers' liability insurance as required under -the laws of the. state and shall hold.the city <br />free and harmless from all liability from any cause that may arise by any reason of injuries to <br />any employee of the licensee. <br />(2) In the event the licensee is the .sole .proprietor and has not elected to prrovide workers' <br />compensation insurance, the licensee shaft be required to. execute and submit an affidavit of <br />sole proprietorship: in a form satisfactoryto the city or provide satisfactory evidence of self <br />insurance according to law. <br />(3) The license applicant must present proof of insurance on each vehicle with an insurance <br />company licensed to do business in the state, The minimum limits of the policy shall. be <br />$200,OOO,00 for bodily injuries. or death of one person, and $600,000.OQ:for any one accident <br />
